I hereby release and hold harmless (1) Love Our Schools and its Board of Directors (2) Lafayette Parish School System, and (3) the sponsors and supervisors of all related activities, from all liability for any injury I may suffer (including any injury caused by negligence) in conjunction with this volunteer activity. I also certify that I am in good health and able to participate in the program activities. I permit that I may be filmed and photographed during this event, and that my name, image, and likeness can be used in print and digital media to make it visible to the public. I certify that I am 18 years of age or older and am competent to contract my own name insofar as the above is concerned, or that I am an authorized guardian completing this for someone under the age of 18. I have read the foregoing release, authorization and agreement, before affixing my signature below/checking the box on the online volunteer portal and warrant that I lawfully understand the contents thereof.
